How/when do I get a PR rep in BWT?

Anyone know when I get a PR rep? Sorry if this was already posted; I looked and searched for the answer but I was unable to find it.

Is it based on fans or stars?

It's based on achieving the other "plot points", and then stars in the appropriate city.

As you play through sets it will occasionally give you hints in the post-game stats regarding the plot.

For example:

"You will be eligible for bodyguards once you earn more stars in Stockholm!"

PR Firm, Bodyguards, etc are based on the number of stars you obtained. From the location screen, if you press the yellow fret or bass pedal for drums you'll call your manager and he'll tell you what you need to do. But basically just run around and collect stars.
