I'm curious about how the game manages the difficulty for vocals. Can anyone explain the difference between say, Medium and Expert?

The amount of "score" you get on each word is more strict. So on easy, when you are singing slightly off pitch and your "note arrow" (all made up terms) is just barely on the green, you get full score per word. On expert you have to be right in the middle to get the full score, the outside edges get you nothing. The score per phrase determines your "Messy", "Good" or "Awesome" rating, which affects your rock meter.
That's the best way I can describe it.

How close you have to be to the note for it to count. Harder difficulties, you have to be less sharp or flat.

From what I gathered by playing, it basically works with a gauge that fills up when you hit the correct notes. The higher the gauge gets the better your phrase is ranked, and your points and life bar are attributed/moved accordingly.
So for the difficulty, they basically make the gauge easier to fill. It will take less time on the correct notes to get a better phrase ranking, thus making scoring/surviving easier.

So, in Expert mode do you have to sing a perfect cover of the song your trying to play?

Sort of, but it's hitting the notes rather than putting on an impression of the singer. It's more forgiving that trying to look for perfect, but I guess that's because that imperfection is an integral part of Rock Music.
