They are listed right in the achievements screen inside the game (and are not secret) but I believe they are save a bandmate from failing, get 100% in the end solo on drums, and get 100% in the end solo on guitar. (Both on expert.)

The guitar solo one for The End can be done on Hard or Expert.

Is the "you saved a bandmate from failing" one really "you brought back a bandmate that failed out"? Or do you have to kick it in when your bandmate is in the red but not failed?

Saved a bandmate from failing the whole group out. So activate it to bring them back in after they personally failed out. :)

Yes. Most games for the X360 have "secret" achievements that you can't see by looking at your Achievement list on the Xbox dashboard.
