Can someone tell me what determines when you stop getting fans while on BWT on a particular level of difficulty and have to move up to the next level to get fans again?

It's the number of fans you currently have versus the lowest level difficulty of anyone in your band who plays a song. So, if you have anyone playing on Easy, then you will not be able to get more fans beyond the cap set for Easy. As for what the exact numbers are now after they were recently changed, no idea. But you will be able to get into the Hall of Fame and Endless Setlist with everyone playing on Medium difficulty.

At least one million, since they noted that 360 owners could get the 1 million fans achievement on Hard.
