Well... I can't say for sure, but maybe. I know my hand sometimes hurts after long sessions. It's a pretty good work out, as well.

I think it might, I remember when I fisrt started playing rock band my wrists hurt pretty bad. The next day when i was playing I didnt feel any pain. So I'm sure it probly does a little.

I haven't noticed any callouses, though i've only been playing regularly since february, so maybe I haven't had enough exposure

Playing real drums definitely will build up callouses on your palms and fingers. Your body builds callouses to protect against damage, and most of the damage from playing drums is from the stick vibrations rubbing against your hands when you hit cymbals.

Playing RB drums doesn't involve that kind of jarring movement, so probably won't toughen up your hands all that much. Maybe a little, but not nearly to the same degree.

Honestly, if you're developing callouses from playing RB drums, you're either 1) hitting them too hard, or 2) holding the sticks way too tightly. Or a combination of both.

sorry, i play real drums and am drunk and not thinking and you should play real drums and you'll have callouses too and we'll box...
but not together... ew.

Well, my hand has callouses, which could either be from drumming on expert, or from cooking for a living... you tell me.

i 5-starred all of the on-disc songs, but i'm stuck on macaroni & cheese in the kitchen...

I've been RB drumming since game came out...my foot doesn't get nearly as sore as it use to. I can now play drums for hours, I use to have to switch after a few songs. On the hands thing, I've never really had a problem with callouses. Think its time for me to go boxing with my girlfriend :D
