A guitar is meant to be played with the fattest, lowest strings up top, and the thinnest, highest strings on bottom. On a guitar that is otherwise perfectly symmetric, which way you have to hold the guitar to make the strings line up properly dictates whether that guitar is a righty or lefty.
On guitars that aren't perfectly symmetric there are other clues, but even on those the fat strings still will be physically above the thin strings.
