I HAVE to go with "Quote Unquote" by Mr. Bungle. Not only is the video just creepy as all hell, but the song makes you feel as if you are in a haunted circus with clowns holding hatchets walking around and toy dolls hanging from the ceiling.... thats just what I picture.

props for the walker brothers mention...
as someone who purposely seeks out musical outsiders and oddities, one of the creepiest things i've ever heard is jandek...
though none of the songs i initially searched for were on youtube... here's a pretty good one... just ignore the fan-made video...

I always thought "Let Me Take You Home Tonight" (Boston) was incredibly creepy, even thought I love it to death.
Every Breath You Take pretty much takes this competition running away, though, unless you go into songs that are deliberately trying to be as creepy as possible.
